Job summary

Imagine Pediatrics is seeking a Patient Engagement Associate II to enroll and onboard children and caregivers into our program. You will work with a tight-knit team, applying critical thinking and outreach to connect families with ongoing support and care coordination.

The role emphasizes field readiness, bilingual Spanish, and strong communication to address barriers to enrollment in a fast-growing pediatric care model.

Qualifications

• 2+ years in care coordination, community health, patient engagement, customer service, or related fields.
• Experience in a centralized contact center environment preferred.
• Experience navigating social services and resourcing for families.
• Strong documentation skills; CRM familiarity is a plus.
• Ability to work in a collaborative, fast-paced startup setting.

Responsibilities

• Research attributed patients to identify enrollment barriers and outreach opportunities.
• Conduct telephonic outreach to caregivers to enroll children into care.
• Reach out to healthcare professionals involved with attributed patients and caregivers.
• Leverage internal and external resources to support enrollment and care coordination.
• Handle inbound caregiver calls as part of the enrollment process.
• Learn SSI program basics and outreach to eligible caregivers.
• Collaborate with Providers to support enrollment efforts on-site when appropriate.
