Job summary

CareTalk Health is a fully remote, nationwide virtual medical practice focused on Medicare Advantage plans. The Patient Support Specialist acts as the first point of contact, coordinating care teams, confirming readiness for visits, and ensuring smooth handoffs to clinicians from a home-based setting.

The role requires healthcare or customer service experience, strong digital fluency across Zoom, Teams, and EHR/CRM tools, and excellent communication.

Qualifications

• Experience in a healthcare or customer service setting required.
• Strong digital fluency and comfort with navigating multiple platforms (Zoom, Teams, EHR/CRM tools).
• Excellent interpersonal and communication skills.
• Ability to multitask, prioritize, and maintain calm in a fast-paced virtual environment.

Responsibilities

• Care Team Coordination: serve as central coordinator for multiple care teams, ensuring patients connect with the right clinician at the right time.
• Patient Intake and Check-In: initiate outbound calls to confirm appointments, verify identity, and ensure readiness for the visit.
• Communication and Documentation: maintain accurate documentation of patient outreach, intake, call routing, and handoff activities.
• Technical Navigation & Support: troubleshoot minor technical issues and escalate as needed.
