Job summary

Carewell is seeking a remote Care Concierge to guide members through enrollment, welcome visits, and ongoing care navigation. You will coordinate with providers, pharmacies, and community resources while maintaining clear documentation and ensuring compliance.

The role offers 100% remote work, EST-hours, and a chance to shape a growing program by adapting to real-time needs and feedback from the care team.

Qualifications

• 1+ years of member-facing healthcare experience.
• Willingness to work Monday–Friday, 9am–6pm EST.
• Must be located in Alberta, Canada (authorized to work in Canada).
• Bilingual (Spanish preferred; other languages a plus).

Responsibilities

• Enroll prospective program participants via outbound outreach.
• Conduct welcome visits with newly enrolled members.
• Coordinate ongoing care navigation and resource connections.
• Build rapport with older adults and address concerns.
• Document interactions accurately in real time.
• Navigate across platforms (care navigation system, CRM, Google Workspace).
