Job summary

Carewell is seeking a Remote Care Concierge to support older adults with high-risk chronic conditions through proactive care coordination, education, and advocacy. This is a remote role requiring empathetic engagement, clear communication, and the ability to manage a sizable caseload while partnering with clinical teams.

You will navigate patients through the healthcare system, coordinate appointments, address barriers to care, and connect them with community resources to improve health

Qualifications

• Must have CMA or RMA credential.
• 1+ year of remote care coordination experience.
• Experience with Medicare-enrolled or dual-eligible populations.
• Strong ability to build trust with older adults.
• Proficient with EHR systems, care management platforms, and CRMs.

Responsibilities

• Serve as primary contact for enrolled patients with compassionate outreach.
• Conduct regular check-ins to assess health status and care needs.
• Manage caseload of 75–150 patients.
• Coordinate with family caregivers to support care.
• Navigate healthcare system and coordinate appointments.
• Escalate clinical concerns to LVN/clinical team.
• Document all patient interactions in real time.
