Job summary

Carewell is seeking a Remote Care Concierge to support older adults with chronic conditions through proactive care coordination, education, and advocacy. You will navigate patients through the healthcare system, coordinate with clinicians, and help them access medications and services while building lasting patient relationships.

You will work remotely, manage a caseload, and collaborate with clinical teams to improve health outcomes.

Qualifications

• Must have an active CMA or RMA credential.
• 1+ year of remote care coordination experience (CCM, RPM, care navigation).
• Experience working with Medicare-enrolled or dual-eligible populations.
• Ability to build trust with older adults and manage chronic conditions.
• Strong understanding of care coordination principles and healthcare navigation.
• Comfort discussing chronic conditions, medications, and treatment plans with patients.
• Proficient with EHRs, care management platforms, CRMs, and digital tools.
• Self-directed, metric-aware, and able to manage a caseload remotely.
• Adaptable to ambiguity and rapid iteration in a startup environment.
• Proven remote work capability and reliable home workspace.
• High school diploma required; associate's or bachelor's preferred.
• Must reside in FL, GA, NC, TN, or TX.

Responsibilities

• Serve as the primary remote care concierge for enrolled patients, building trust through proactive outreach.
• Coordinate appointments, facilitate communication between providers, and ensure care plans are understood.
• Maintain a caseload and prioritize outreach based on acuity and gaps in care.
• Engage family members and caregivers to support coordinated care.
• Use multiple outreach methods while adapting to patient needs.
