Job summary

TailorCare, Inc. is a remote-first company with a Nashville corporate office, seeking a Clinical Navigator (DPT) to deliver high-quality navigation through telephonic and video interactions.

You will engage members, assess needs, build trust, and support adherence to evidence-based care plans in a fast-paced, data-driven setting. The ideal candidate has a DPT or MPT, 3+ years in musculoskeletal care or care navigation, and active licensure in applicable states.

Qualifications

• Doctor of Physical Therapy (DPT) or Masters in Physical Therapy (MPT).
• Bilingual - English and Spanish.
• Current unrestricted physical therapy license in Georgia, Texas, Colorado and/or Indiana (Compact License preferred).
• Willingness to obtain additional state licenses as TailorCare expands.
• Minimum 3 years of experience in physical therapy, musculoskeletal care, or care navigation.

Responsibilities

• Serve as the primary navigator for an assigned member population in a high-volume, contact-center environment.
• Conduct comprehensive clinical intake assessments for MSK conditions and goals.
• Deliver navigation via telephonic and video interactions to support outcomes.
• Maintain high volume of member interactions with empathy and clinical rigor.
• Educate members on options, resources, and care plans to support informed decisions.
• Monitor progress and escalate when needed to optimize outcomes.
• Document all interactions within required timelines and ensure compliance.
