Job summary

TailorCare seeks a licensed Doctor of Physical Therapy to deliver high-quality clinical navigation in a fast-paced, virtual care environment. The Clinical Navigator will engage members via calls, video visits, and digital touchpoints to assess needs, build trust, and support adherence to evidence-based care plans.

The role emphasizes clinical expertise, operational excellence, and the ability to meet engagement and quality metrics in a remote-first setting with travel up to 10% for onsite

Qualifications

• DPT or MPT with an active state license.
• Licensed to practice in Georgia, Texas, Colorado and/or Indiana (Compact preferred).
• Minimum 3 years of experience in physical therapy, musculoskeletal care, care navigation, telehealth.

Responsibilities

• Serve as primary navigator for an assigned member population in a contact-center environment.
• Conduct comprehensive clinical intake assessments to evaluate MSK conditions and goals.
• Deliver navigation via telephonic and video-based interactions, tailoring engagement to needs.
• Educate members on treatment options, clinical recommendations, and resources to support informed decisions.
• Proactively follow up with members to drive adherence and engagement.
• Collaborate with interdisciplinary care teams to ensure coordinated care.
• Maintain documentation & compliance with protocols and policies.
