Job summary

TailorCare is seeking a Clinical Navigator (DPT/MPT) to deliver high-quality, AI-assisted care navigation in a remote-first model. You will engage members by phone and video, assess MSK needs, and guide care plans to improve outcomes from intake to program completion.

The role emphasizes collaboration with interdisciplinary teams, documentation accuracy, and adherence to evidence-based MSK pathways, with opportunities to shape future virtual therapy programs from our Nashville HQ.

Qualifications

• DPT or MPT required and license in good standing.
• Active PT license in GA, TX, CO or IN (Compact preferred).
• Willingness to obtain/maintain additional state licenses as TailorCare expands.
• Minimum 3 years in PT, MSK care, care navigation, telehealth, or related clinical settings.
• Demonstrated success in high-volume, performance-driven environments.
• Strong communication, motivational interviewing, and patient engagement skills.
• Experience using technology and data to improve workflow efficiency.
• Ability to work independently with accountability to productivity and quality.

Responsibilities

• Serve as the primary navigator for an assigned member population in a contact center.
• Conduct comprehensive clinical intake assessments for MSK conditions and goals.
• Deliver clinical navigation through telephonic and video interactions.
• Maintain a high volume of member interactions with personalized, empathetic care.
• Educate members on treatment options and care plans to support decisions.
• Proactively follow up to drive adherence and improve engagement outcomes.
• Document interactions and outcomes within established timelines.
