Job summary

TailorCare seeks a licensed Doctor of Physical Therapy to serve as the Clinical Navigator, delivering high-quality navigation through telephonic and video interactions in a fast-paced, metrics-driven clinical operations environment.

The role requires clinical expertise, operational excellence, and strong communication to engage members, build trust, and support adherence to evidence-based care plans.

Qualifications

• Doctor of Physical Therapy (DPT) or Masters in Physical Therapy (MPT) is required.
• Bilingual in English and Spanish is required.
• Current unrestricted PT license in GA, TX, CO and/or IN (Compact license preferred).
• Willingness to obtain additional state licenses as TailorCare expands.
• Minimum 3 years of experience in PT, MSK care, care navigation, telehealth, or related setting.

Responsibilities

• Serve as the primary navigator for an assigned member population from intake to program completion.
• Conduct comprehensive clinical intake assessments for MSK conditions and goals.
• Deliver navigation via telephonic and video interactions with evidence-based care.
• Maintain high daily member interactions with empathetic, clinically sound experience.
• Educate members on treatment options and resources for informed decisions.
• Follow up to drive adherence and remove barriers to care.
