Job summary

Motion MSK is seeking a licensed Physical Therapist Care Navigator to guide patients through the MSK care journey via telephonic support. You will triage, educate, and partner with providers to ensure the right care at the right time in a remote environment.

You will build trusted relationships, conduct intake assessments, and coordinate care across the ecosystem, using evidence-based guidance and motivational interviewing to promote engagement and optimal outcomes.

Qualifications

• DPT/Physical Therapist with 5+ years orthopedic/MSK experience.
• Active, unrestricted PT license in Oregon; PT Compact preferred.
• Strong clinical judgment and triage skills; telephonic environment experience.

Responsibilities

• Serve as the clinical escalation point guiding triage recommendations and care pathways.
• Conduct evidence-based intake assessments and determine appropriate care pathways.
• Provide ongoing clinical guidance, education, and shared decision-making support.
• Document all patient interactions in EMR and ensure care coordination.
