Job summary

TailorCare, Inc. is seeking a licensed Doctor of Physical Therapy to serve as a Clinical Navigator, delivering high-quality telephonic and video-based clinical navigation for members with musculoskeletal conditions.

The role supports a remote-first culture with a Nashville office, requiring strong engagement, operational excellence, and experience in telehealth or digital MSK programs. Travel up to 10% may be required.

Qualifications

• DPT or MPT degree required.
• Active/unrestricted PT license in GA, TX, CO, or IN.
• Willingness to obtain additional state licenses.
• ≥3 years in PT, MSK care, care navigation, or telehealth.
• Strong communication and patient engagement skills.
• Comfort with technology-assisted care and data usage.
• Willing to travel up to 10%.

Responsibilities

• Serve as primary navigator for an assigned member population.
• Conduct comprehensive clinical intake assessments for MSK conditions and goals.
• Deliver clinical navigation via telephonic and video-based interactions.
• Maintain a high volume of member interactions with empathy and clinical soundness.
• Educate members on treatment options and resources to support informed decisions.
• Proactively follow up to drive adherence and engagement.

Education

Doctor of Physical Therapy (DPT) or Masters in Physical Therapy (MPT)
