Job summary

Cadence is hiring a Clinical Navigator (LPN/LVN) to deliver virtual chronic care support for Medicare patients enrolled in Cadence programs. You will conduct outreach, provide coaching, and coordinate care using Cadence tools and AI-assisted support, while maintaining clinical judgment.

The role is full-time and remote in the United States, with standard Pacific time business hours. Fluency in Spanish is preferred and a compact multi-state license is required.

Qualifications

• Active compact multi-state LPN/LVN license required.
• At least 5 years of clinical experience as an LPN/LVN.
• Remote work readiness with stable high-speed WiFi required.

Responsibilities

• Conduct outbound patient outreach and provide coaching for chronic condition management.
• Educate and coach patients on behavior modification, nutrition, activity, and self-management.
• Assess patient health status, readiness to change, and adherence to plans.
• Educate on disease management, medication adherence, symptoms, and prevention per Cadence policies.
• Document interactions and outcomes in the Cadence platform; identify barriers to care.
