Job summary

Cadence is hiring a Clinical Navigator (Licensed Practical Nurse) to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. This part-time, remote role requires an active LPN/LVN license and at least 5 years of clinical experience, with preference for Spanish fluency.

You will coach patients, coordinate care, and document interactions within Cadence’s platform, collaborating closely with clinicians.

Qualifications

• Active LPN/LVN license in multi-state where you reside.
• Minimum 5 years of clinical experience as an LPN/LVN.
• Remote work readiness with stable high-speed WiFi.
• Active California LVN license.
• Spanish language proficiency preferred.
• Experience with AI-assisted clinical tools is a plus.

Responsibilities

• Perform outbound patient outreach and one-on-one coaching for chronic conditions.
• Help patients execute personalized care plans and goals with education and support.
• Conduct assessments of health status and readiness to change.
• Educate patients on disease management, adherence, and prevention; follow Cadence policies.
• Monitor progress, adherence, and outcomes via remote tools; document interactions.
