Job summary

Cadence is looking for a Clinical Navigator (Licensed Practical Nurse) to provide virtual chronic care support for Medicare patients. Responsibilities include patient outreach, care coordination, and documentation of interactions in the Cadence platform.

This full-time remote role requires a compact multi-state LPN/LVN license and at least 5 years of clinical experience. Preferred qualifications include fluency in Spanish and experience in managing chronic conditions.

Qualifications

• An active compact multi-state LPN/LVN license is required.
• At least 5 years of clinical experience as a Licensed Practical Nurse is needed.
• Fluency in Spanish is preferred.

Responsibilities

• Conduct patient outreach and provide one-on-one coaching.
• Help patients execute care plans to achieve their goals.
• Monitor patients’ progress through regular check-ins.
