Cadence, a clinical AI company, is hiring a Clinical Navigator (LPN) to deliver virtual chronic care support for Medicare patients enrolled in Cadence programs. You will conduct proactive outreach, coaching, and care coordination to close gaps in care, while documenting interactions in the Cadence platform.

The role is full-time and remote, based in the United States, with standard hours in Pacific time. Requires an active LPN/LVN license and 5+ years of clinical experience, plus remote-work

Qualifications

• Active compact multi-state LPN/LVN license is required.
• Minimum 5 years of clinical experience as an LPN/LVN.
• Remote work readiness with stable high-speed WiFi is required.
• Active CA state LVN license is required.
• Fluency in Spanish preferred.

Responsibilities

• Conduct outbound telephonic patient outreach and coaching for chronic conditions.
• Provide education and coaching on behavior change, nutrition, activity, and self-management.
• Perform comprehensive assessments of health status and readiness to change.
• Educate patients on disease management and medication adherence per Cadence policies.
• Monitor progress and health outcomes through remote check-ins.
• Document interactions and coordinate care using Cadence platform and tools.
