Job summary

Cadence is seeking a Clinical Navigator (LPN/LVN) to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. This full-time, remote role operates Monday–Friday, with Pacific or Mountain Time schedules.

You will conduct outbound outreach, coach patients, and coordinate care under clinical supervision. Applicants must hold an active multi-state LPN/LVN license, have at least 5 years of clinical experience, and be prepared for remote work with reliable WiFi.

Qualifications

• Active compact multi-state LPN/LVN license required.
• Minimum 5 years of clinical experience as an LPN/LVN is required.
• Remote work readiness with stable high‑speed WiFi is required.
• An active CA state LVN license is highly preferred.
• Fluency in Spanish is strongly preferred.
• Experience providing patient education and support for chronic conditions is preferred.
• Familiarity with AI-assisted clinical tools and using them to support care delivery is a plus.

Responsibilities

• Conduct outbound telephonic patient outreach and provide one‑on‑one coaching to patients managing chronic conditions.
• Help patients execute personalized care plans with education and coaching on behavior modification and self‑management.
• Conduct comprehensive assessments of health status, lifestyle, and readiness to change.
• Educate patients on disease management, medication adherence, symptoms, and prevention strategies.
• Monitor progress and outcomes via regular check‑ins and remote monitoring tools.
• Document interactions and outcomes in the Cadence platform and route barriers to care as needed.
• Collaborate with clinicians and patient support teams in real time using Cadence Platform, Google Workspace, and Slack.
