Job summary

Cadence is hiring a Clinical Navigator (Licensed Practical Nurse) to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. You will conduct proactive outreach, coaching, and care coordination to close care gaps.

The role requires LPN/LVN licensure, 5+ years clinical experience, and the ability to work remotely from the United States. Compensation is hourly with full benefits and remote work setup provided.

Qualifications

• Active compact multi-state LPN/LVN license in the state of residence.
• Minimum 5 years of clinical experience as an LPN/LVN.
• Remote work readiness with stable high-speed WiFi.
• Active CA state LVN license is required.
• Spanish language fluency is preferred.
• Experience educating and supporting patients with chronic conditions is preferred.
• Familiarity with AI-assisted clinical tools and ability to discuss AI support versus human judgment.

Responsibilities

• Conduct outbound telephonic patient outreach and provide one-on-one coaching and support to patients with chronic conditions (e.g., type 2 diabetes, hypertension, CVD).
• Assist patients in executing personalized care plans through education and coaching on behavior change, nutrition, activity, and self-management using evidence-based methods (motivational interviewing, SMART goals).
• Perform comprehensive assessments of health status, lifestyle, nutrition, and readiness to change.
• Educate patients on disease management, medication adherence, symptoms, and prevention; perform care delivery tasks per Cadence policies under clinical supervision.
• Monitor progress, treatment adherence, and health outcomes via regular check-ins and remote monitoring tools.
• Document patient interactions, outcomes, and follow-up plans in the Cadence platform; identify barriers to care and connect patients with resources or internal support.
• Collaborate in real-time with clinicians and patient support teams using Cadence Platform, Google Workspace, and Slack; provide feedback to improve workflows in a fast-paced environment.
