Job summary

Cadence is hiring a Clinical Navigator (Licensed Practical Nurse) to deliver virtual chronic care support for Medicare patients. You will perform structured check-ins, coaching, care coordination, and resource linkage, documenting in the Cadence platform and collaborating with the clinical team to ensure patient engagement and outcomes.

The role is full-time and remote within the United States, with core hours in Pacific Time.

Qualifications

• Active compact multi-state LPN/LVN license required.
• At least 5 years of clinical experience as an LPN.
• Remote work readiness with stable high-speed WiFi.
• Active CA state LVN license required.
• Spanish fluency preferred.
• Experience educating and supporting patients with chronic conditions.
• Familiarity with AI-assisted clinical tools and discussing AI use with providers.

Responsibilities

• Conduct outbound telephonic patient outreach and one-on-one coaching.
• Help patients execute personalized care plans with education and coaching.
• Perform comprehensive health and lifestyle assessments.
• Educate on disease management, adherence, symptoms, and prevention.
• Monitor progress and health outcomes via check-ins and remote tools.
• Document interactions and coordinate with resources or internal supports.
• Collaborate with clinicians and support teams using Cadence Platform, Google Workspace, and Slack.
