Job summary

Cadence Health is hiring a Clinical Navigator (Licensed Practical Nurse) to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs.

As a Clinical Navigator you will conduct proactive outreach, coach patients on behavior change, and coordinate care by closing gaps, addressing barriers like access, transportation, or medications. You will document interactions and collaborate with clinicians in a remote, fast-paced environment.

Qualifications

• Active compact multi-state LPN/LVN license required.
• At least 5 years of clinical experience as an LPN.
• Remote work readiness (stable high-speed WiFi) required.
• Active CA state LVN license required.
• Spanish fluency preferred.
• Experience educating and supporting patients with chronic conditions.
• Familiarity with AI-assisted clinical tools and able to discuss AI roles vs human judgment.

Responsibilities

• Conduct outbound telephonic patient outreach and provide one-on-one coaching for chronic conditions.
• Support patients in executing personalized care plans with education and goal-setting.
• Assess health status, readiness to change, and lifestyle factors.
• Educate on disease management, medication adherence, and prevention.
• Monitor progress and health outcomes via remote tools and follow-ups.
• Document interactions in Cadence platform and connect patients with resources.
• Collaborate with clinicians and support teams to improve workflows in a fast-paced setting.
