Job summary

Cadence, a clinical AI company, is hiring a Clinical Navigator (Licensed Practical Nurse) to deliver virtual chronic care support for Medicare patients enrolled in Cadence programs. You will coach, assess, and coordinate care, documenting interactions in cadence software while collaborating with the clinical team to ensure patients feel supported and informed.

This full-time remote role requires an active LPN/LVN license, 5+ years experience, and remote readiness.

Qualifications

• An active compact multi-state LPN/LVN license in the state where you reside is required.
• At least 5 years of clinical experience as a Licensed Practical Nurse is required.
• Remote work readiness (including stable high-speed WiFi) is required.
• An active CA state LVN license is required.
• Fluency in Spanish is preferred.
• Experience with AI-assisted clinical tools and ability to discuss AI support vs human judgment.

Responsibilities

• Conduct outbound telephonic patient outreach and provide one-on-one coaching and support to patients managing chronic conditions.
• Help patients execute personalized care plans with education and coaching on behavior modification, nutrition, activity, and self-management.
• Conduct comprehensive assessments of health status and readiness to change.
• Educate patients on disease management, medication adherence, and prevention strategies.
• Monitor progress and health outcomes through regular check-ins and remote monitoring tools.
• Document patient interactions and follow-up plans in the Cadence platform; connect patients with resources as needed.
• Collaborate with clinicians and patient support teams using Cadence Platform, Google Workspace, and Slack.
