Job summary

Cadence is seeking a Clinical Navigator (Licensed Practical Nurse) to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. This full‑time, remote role is based in the United States with standard M‑F hours in PT/Mountain time zones.

You will conduct outbound outreach, coach patients on behavior change, educate on disease management, and coordinate care under clinical supervision. Remote work readiness and a multi‑state LPN/LVN license are required.

Qualifications

Active multi-state LPN/LVN license required.
Minimum 5 years of clinical experience as an LPN/LVN.
Remote work readiness with stable high‑speed WiFi required.
CA LVN license preferred.
Spanish fluency preferred.
Experience educating patients with chronic conditions preferred.
Familiar with AI‑assisted clinical tools and able to discuss AI support vs. human judgment.

Responsibilities

Outreach to patients by phone; provide one‑on‑one coaching for chronic conditions (e.g., diabetes, hypertension, CVD).
Guide patients to execute personalized care plans; educate on behavior modification, nutrition, activity, and self‑management.
Conduct health status assessments and readiness to change.
Educate on disease management, medication adherence, symptoms, and prevention; follow Cadence policies under supervision.
Monitor progress and health outcomes via remote monitoring tools.
Document interactions, outcomes, and follow‑ups; connect patients to resources or internal support.
