Job summary

Cadence is hiring a Clinical Navigator (LPN) to deliver remote chronic care support for Medicare patients enrolled in Cadence care programs. You will conduct outbound outreach, coach patients, and coordinate care under appropriate clinical supervision.

Ideal candidates hold an active multi-state LPN license with 5+ years of nursing experience and remote-work readiness. Spanish fluency is a plus; AI-assisted clinical tools may be used to support care delivery.

Qualifications

• Active LPN/LVN license (multi-state) is required.
• Minimum 5 years of clinical experience as an LPN.
• Remote-work readiness and reliable high‑speed internet required.
• Fluency in Spanish is preferred.

Responsibilities

• Outbound telephonic outreach and one‑on‑one coaching for chronic conditions.
• Help patients execute personalized care plans with education and behavior modification.
• Conduct comprehensive health status assessments and readiness to change.
• Educate on disease management, medication adherence, and prevention under clinical supervision.
• Monitor progress and outcomes using remote monitoring tools.
• Document interactions in the Cadence platform and connect patients with resources.
