Job summary

Vynca Inc. is seeking an Intake Coordinator to support the ECM Sales/Marketing and Clinical Outreach teams in enrolling patients into our programs.

The role emphasizes patient relationship-building, medical chart review, and enrollment coordination in a fast-paced, remote setting. The ideal candidate has at least 2 years of intake or admission experience, a CMA/CCMA certificate, and familiarity with palliative/home health contexts.

Qualifications

• Minimum of 2 years of intake or admission experience in a high call volume environment.
• CMA/CCMA certificate issued by a recognized group.
• Prior experience in a remote role with a quiet, secure workspace.
• 2+ years in palliative care, home health, hospice, or discharge planning preferred.
• 2+ years working with large safety net hospitals in the region preferred.
• Monday-Friday 8:30am-5:00pm PT schedule, with possible evenings/weekends.
• Bilingual English/Spanish and cultural competency a plus.
• Experience with Google Suite and Salesforce.

Responsibilities

• Facilitates intake, admission, and assists with outreach to enroll patients.
• Provides accurate ongoing assessment of patient status in admission.
• Maintains databases, logs, census sheets, and contact lists.
• Performs insurance eligibility checks and assigns clients to specialists.
• Works cross-functionally with sales, outreach, care delivery and RCM to manage enrollment workflow.
• Records all interactions in the operating system and participates in team meetings.
• Responds to calls and emails and helps meet departmental KPIs.
