Qualifications

• Minimum of 2 years of intake/admission experience in high call volume.
• CMA/CCMA certificate issued by a recognized group.
• Remote work requires a quiet, secure workspace.
• 2+ years in palliative care, home health, hospice, or discharge planning.
• Experience with large safety net hospitals preferred.
• Ability to work Mon-Fri 8:30am-5:00pm PT; may include evenings/weekends.
• Strong cultural competency with target population.
• Experience with Google Suite and Salesforce.
• Training alongside social workers or nurses is a plus.
• Bilingual English/Spanish preferred.

Responsibilities

• Facilitate intake and enrollment processes for new patients.
• Perform insurance eligibility checks and assign to Clinical Outreach.
• Maintain databases, logs, and census sheets.
• Collaborate with Sales, Outreach, Care Delivery, and RCM.
• Document all interactions in the system.
• Participate in staff and team meetings.
