Vynca Inc. is seeking an Intake Coordinator to support ECM Sales/Marketing and Clinical Outreach by organizing and enrolling patients in our programs.

This 1099 contract role involves coordinating intake, reviewing medical charts, and building relationships with teams and families. The ideal candidate has at least 2 years in intake/admissions, CMA/CCMA certification, and comfort working remotely with a quiet home office.

Qualifications

• Minimum 2 years of intake/admission experience in a high call volume environment.
• CMA/CCMA certificate issued by a recognized group.
• Prior experience in a remote role with a quiet, secure workspace for privacy.
• 2+ years in palliative care, home health, hospice, or discharge planning; highly preferred.
• 2+ years with large safety net hospitals in the region; preferred.
• Bilingual English/Spanish is highly preferred.

Responsibilities

• Facilitates intake, admission, and outreach for incoming patients.
• Provides accurate ongoing assessment of patient status at admission.
• Maintains databases, logs, census sheets, and contact lists; updates resources and schedules.
• Performs insurance eligibility checks and assigns clients to the Clinical Outreach Specialist.
• Works cross-functionally with sales, outreach, care delivery and RCM to enroll patients.
• Documents all enrollment interactions in the operating system.
• Participates in staff, census, and Outreach Team meetings; meets KPIs.
