Job summary

Vynca Inc. is seeking an Intake Coordinator to support ECM Sales/Marketing and Clinical Outreach Specialists in enrolling patients into our programs. The ideal candidate is personable, professional on calls, and comfortable reviewing medical charts with a customer-centric approach.

This 1099 contract may convert to full-time; we welcome applicants from the listed states and offer remote work with a Pacific Time-focused schedule.

Qualifications

Minimum of 2 years of intake/admission experience in a high call volume environment.
CMA/CCMA certificate issued by a recognized group.
Remote role requires a quiet, secure workspace to protect member privacy.
2+ years of experience in palliative care, home health, hospice, or hospital discharge planning or case management.

Responsibilities

Facilitate intake, admission, and outreach for incoming patients.
Provide accurate ongoing assessment of patient status in the admission process.
Maintain databases, logs, census sheets, and contact/resource lists.
Complete insurance eligibility checks and assign clients to the Clinical Outreach Specialist.
Collaborate cross-functionally with sales, outreach, care delivery and RCM to support enrollment workflow.
Document all enrollment interactions in the operating system.
Attend staff, census and outreach meetings; participate in weekly Outreach Team meetings.
Meet or exceed departmental KPIs and support organizational needs.
