Qualifications

• Case management and discharge planning experience preferred.
• 2 years experience in behavioral health, social services or related field.
• Managed Care experience preferred.
• Ability to travel within a designated geographic area for in-person activities.
• Excellent analytical and problem-solving skills.

Responsibilities

• Evaluate member needs/eligibility and recommend a plan of care.
• Identify high-risk factors and coordinate appropriate referrals.
• Coordinate and monitor care plan activities and progress.
• Consult with case managers and other health programs to overcome barriers.
• Engage with colleagues in team meetings and mentor peers.
• Assist members with appointments and accessing benefits.
