Responsibilities

• Evaluate Members' needs/eligibility and recommend care resolution.
• Identify high risk factors and coordinate care plan activities.
• Monitor care plan progress and outcomes.
• Collaborate with Medical Directors and health programs to improve care quality.
• Escalate quality of care issues through established channels.
• Engage members to participate in healthcare decisions and healthy lifestyles.
• Provide coaching and information to empower members.
