Qualifications

• 2+ years experience in Behavioral Health or Social Services.
• Willing to travel 50-75% in Cook County, IL.
• Reliable transportation required.
• 2+ years of electronic documentation experience.

Responsibilities

• Evaluate Members: assess needs/eligibility and recommend care plan actions.
• Identify high risk factors and arrange appropriate referrals.
• Coordinate and monitor care plan activities and progress.
