Qualifications

• 2+ years experience in Behavioral Health or Social Services.
• Must travel 50-75% around Cook County, Illinois with reliable transportation.
• 2+ years electronic documentation experience and Microsoft Office applications.

Responsibilities

• Evaluate members' needs/eligibility and recommend care plan solutions.
• Identify high risk factors and make appropriate referrals to clinical care.
• Coordinate and implement care plan activities and monitor progress.
• Collaborate with medical directors and health programs to improve outcomes.
• Escalate quality of care issues through established channels.
• Negotiate options to meet members' benefits and healthcare needs.
• Engage members to support healthy lifestyle changes and independent decision-making.
• Participate in team meetings and provide peer mentoring/training.
• Support member participation with providers in healthcare decisions.
• Document care using established case management processes.
