Qualifications

• 2+ years experience in Behavioral Health, Social Services or related field.
• Willingness to travel 50-75% around Cook County, IL; mainly westside of Chicago.
• Electronic documentation experience and proficiency with Microsoft Office applications.

Responsibilities

• Evaluate members and assess needs/eligibility; recommend care resolution.
• Identify high risk factors and coordinate care plans.
• Coordinate and monitor care plan activities and progress.
• Consult with care teams to overcome barriers and achieve outcomes.
• Escalate quality of care issues through established channels.
• Negotiate options to meet member benefits and healthcare needs.
• Engage members to promote healthy lifestyle changes.
• Provide coaching and information to empower independent health decisions.
• Participate in team meetings and peer mentoring.
• Assist members in healthcare decision-making with providers.
• Document care using care management processes in compliance with guidelines.
