Qualifications

• Bachelor's degree or master level clinician in behavioral health or human services.
• Experience in case management and discharge planning preferred.
• Managed Care experience preferred.
• Ability to travel within a designated geographic area for in-person case management activities.
• Proficiency with standard corporate software including MS Word, Excel, Outlook and PowerPoint.

Responsibilities

• Evaluate members' needs/eligibility and recommend an approach to case resolution.
• Coordinate and implement care plan activities and monitor progress.
• Identify high-risk factors and refer to clinical case management or crisis intervention as needed.
• Collaborate with multidisciplinary teams to optimize outcomes and address barriers to care.
• Engage with members to promote informed healthcare decisions and healthy lifestyle choices.
