Qualifications

• Case management and discharge planning experience preferred.
• 2 years experience in behavioral health or related field.
• Managed Care experience preferred.
• Willingness to travel within designated geographic area for in-person activities.
• Proficiency with standard corporate software (Word, Excel, Outlook, PowerPoint) and proprietary apps.

Responsibilities

• Evaluate member needs/eligibility and plan care resolution.
• Coordinate and monitor care plan activities and progress.
• Assist with appointment scheduling and accessing benefits/education.
• Engage with members, providers and community organizations to address care needs.
