Qualifications

• 2+ years experience in Behavioral Health, Social Services or related field.
• Willingness to travel 50-75% in Cook County, Illinois with reliable transportation.
• 2+ years of electronic documentation experience and Microsoft Office proficiency.

Responsibilities

• Evaluate members and determine care needs and benefits.
• Identify high risk factors and route to appropriate care management.
• Coordinate and monitor care plan activities and progress.
• Collaborate with teams to improve care quality and outcomes.
• Escalate quality of care issues through proper channels.
• Use negotiation to secure appropriate services for members.
• Engage members to promote healthy behaviors and decisions.
• Provide coaching and support for informed medical choices.
• Participate in team meetings and peer mentoring.
• Assist members in engaging with providers during decision making.
• Document and monitor care in accordance with guidelines.
