Qualifications

• 2+ years experience in Behavioral Health, Social Services or equivalent field.
• Willing to travel 50-75% around Cook County, IL; reliable transportation.
• 2+ years of electronic documentation experience and MS Office experience.

Responsibilities

• Evaluate members’ needs/eligibility and recommend care plan approaches.
• Identify high-risk factors and coordinate appropriate referrals.
• Coordinate and monitor care plan activities and progress.
• Collaborate with Medical Directors and health programs to improve care quality.
• Escalate quality of care issues through established channels.
• Negotiate options to meet member benefits and healthcare needs.
• Engage members to promote healthy lifestyle changes and autonomy.
• Mentor colleagues and participate in team meetings.
• Support member involvement in healthcare decisions.
• Document care processes in compliance with regulations.
