• Minimum 2+ years' experience in Behavioral Health or Social Services.
• Travel 50–75% of the time in IL counties (DuPage, Kane, DeKalb, Kendall).
• Experience with electronic documentation and Microsoft Office applications.

Responsibilities

• Evaluate members' needs and eligibility and recommend care plan approaches.
• Identify high-risk factors and coordinate appropriate referrals.
• Coordinate and monitor care plan activities.
• Collaborate with care teams to improve outcomes and quality of care.
• Support member engagement and educate on benefits.
• Document care processes and ensure compliance.
