St. Luke's University Health Network is seeking a Clinical Triage Specialist (RN) to support the Access Center. The role uses nursing judgment to triage patient calls, assess symptoms, and guide care using established protocols.

Responsibilities include handling inbound calls, updating patient information in the EHR, and providing clinical guidance. The RN will work with POD teams to coordinate care and ensure quality, with home-based work options per policy.

Qualifikationen

• Graduate of an accredited nursing program.
• Active RN licensure in Pennsylvania and New Jersey or other compact state.
• Minimum 2 years clinical experience in a physician office, home health, critical care or emergency room.

• Answers telephones, triages calls, follows clinical protocols.
• Verifies patient demographics and updates electronic health records.
• Escalates clinical patient issues and provides advice based on protocol.
• Responds to escalated electronic messages using clinical judgment.
• Creates a positive patient experience and resolves issues during calls.
• Meets productivity, schedule adherence and quality standards.
• Documents symptoms, nursing assessment, advice and patient responses.
• Coordinates with Access Center teams and practice staff on patient concerns.
• Other duties as assigned.
