St. Luke's University Health Network is seeking a Clinical Triage Specialist (RN) for the Access Center to deliver compassionate patient care and provide clinical triage using established nursing protocols.

The RN will assess symptoms, prioritize care, and guide patients to appropriate services while maintaining accurate documentation in the electronic health record. Qualified candidates are RNs licensed in PA/NJ or other compact states with at least 2 years of relevant clinical experience.

Qualifications

• Graduate of an accredited nursing program with active RN licensure in PA/NJ or other compact state.
• Minimum 2 years of recent clinical experience in physician office, home health, critical care, or emergency room.

Responsabilités

• Answer telephones and triage calls using clinical protocols.
• Verify and update patient demographics in the EHR.
• Escalate clinical issues and provide advice based on protocols.
• Manage escalated electronic patient messages with clinical judgment.
• Provide a positive patient experience and resolve issues during calls.
• Meet productivity, schedule adherence and quality standards in the Access Center.
• Coordinate with PODs and practice teams for patient concerns, refills, or scheduling.
• Document symptoms, nursing assessment, and patient responses accurately.
• Support other Access Center teams to assist patients when needed.
