Job summary

St. Luke's University Health Network is seeking a Clinical Triage Specialist (RN) for the Access Center to deliver an outstanding patient experience and provide clinical support to CTS-MA team members.

The RN will triage patient calls using approved nursing protocols, assess symptoms, and guide patients to appropriate care or self-care, while maintaining documentation and prioritizing care pathways. Remote work from home may be available per policy.

Qualifications

• Graduate of an accredited nursing program.
• Active Registered Nurse licensure in the state(s) specified (PA/NJ or other compact state) as deemed necessary by state law.
• Minimum 2 years recent clinical experience in physician office, home health, critical care and/or emergency room.

Responsibilities

• Answers telephones, prioritizes clinical triage calls, follows clinical protocols, and coordinates services, as needed.
• Verifies patient demographic information and enter/update information into the electronic health record.
• Serves as an escalation point for clinical patient issues and other POD team members requiring clinical support.
• Manages and responds to escalated electronic patient messages, prioritizing and accommodating patients.
• Creates a positive patient experience at every encounter, resolving issues within the scope of the role.
• Meets productivity, schedule adherence, and quality standards as set by the Access Center.
• Uses all resources to assess, prioritize, advise, schedule appointments, or refer calls as necessary.
• Documents symptoms/complaints, nursing assessment, advice, and patient response.
