Job summary

St. Luke's University Health Network is seeking a Clinical Triage Specialist (RN) for the Access Center to deliver compassionate patient support and apply nursing judgment on calls related to direct care.

The role requires an active RN license in PA and/or NJ, with at least 2 years of recent clinical experience in physician offices, home health, critical care, or emergency settings. Remote work is possible under the Network Work from Home Policy.

Qualifications

• Graduate of an accredited nursing program.

Responsibilities

• Answers telephones, prioritizes clinical triage calls, follows clinical protocols, and coordinates services, as needed.
• Verifies patient demographic information and accurately enters the updated information into electronic health record.
• Serves as an escalation point for clinical patient issues and provides clinical advice based on protocols and procedures.
• Manages and responds to escalated electronic patient messages and prioritizes patient needs.
• Creates a positive patient experience and resolves issues at the time of the call within scope of role.
• Meets productivity, schedule adherence, and quality standards in the Access Center.
• Utilizes resources to assess, prioritize, advise, schedule appointments, or refer calls to appropriate facilities.
• Documents symptoms/complaints, nursing assessment, advice provided and patient response.
• Partners with other Access Center teams and practice clinical staff to assist with clinical concerns, medication refills, or scheduling.
• Other duties as assigned.
