Job summary

St. Luke's University Health Network is seeking a Clinical Triage Specialist (RN) to join the Access Center. The CTS will use nursing judgment to triage calls and provide clinical guidance based on approved protocols to support teammates.

Responsibilities include handling calls, updating the EHR, prioritizing cases, and directing patients to the appropriate care. Requires active RN licensure in PA/NJ and at least 2 years of relevant clinical experience.

Qualifications

• Graduate of an accredited nursing program.
• Active RN licensure in PA/NJ or other compact state.
• Minimum 2 years recent clinical experience in physician office, home health, critical care or ER.

Responsibilities

• Answers telephones, prioritizes clinical triage calls, follows clinical protocols, and coordinates services.
• Verifies patient demographic information and updates electronic health record.
• Serves as escalation point for clinical issues and provides clinical advice based on protocols.
• Manages escalated electronic patient messages and prioritizes/cabets as needed.
• Creates a positive patient experience, resolving issues within scope of the role.
• Meets productivity, schedule adherence, and quality standards as set by the Access Center.
• Uses resources to assess, prioritize, advise, schedule appointments, or refer calls to appropriate facility or personnel.
• Documents symptoms/complaints, nursing assessment, advice provided and patient response.
• Collaborates with other Access Center teams and practice clinics for clinical concerns, medications, or scheduling.
• Other duties as assigned.
