St. Luke’s University Health Network is seeking a Clinical Triage Specialist (RN) for the Access Center. You will provide nursing judgment on patient calls, triage using protocols, and coordinate care with practice teams.

The role requires an RN license in PA/NJ or a compact state and at least 2 years of clinical experience in relevant settings. Some duties may be performed remotely per policy.

Qualifikationen

• RN license in PA and/or NJ or compact state as required by law.
• Minimum 2 years clinical experience in physician offices, home health, critical care, or emergency settings.

Aufgaben

• Answer telephones and triage calls using clinical protocols.
• Verify and update patient demographics in the EHR.
• Escalate clinical issues and provide guidance to POD team.
• Respond to escalated messages, prioritizing patient needs.
• Strive to resolve patient concerns during the call when possible.
• Meet productivity, schedule adherence, and quality standards.
• Assess, prioritize, advise, schedule, or refer calls to appropriate care.
• Document symptoms, assessment, advice, and patient responses.
• Collaborate with Access Center teams and practice clinicians on patient concerns.
