Job summary

Fraser in Minnesota is seeking qualified Case Managers to support individuals on CADI/BI and DD waivers. You will advocate for clients, connect them to community resources, and accurately document case notes.

Requirements include a Bachelor's degree (license if in social work), one year of DD experience or willingness to complete Relias I/DD training, and a demonstrated commitment to diversity and inclusion. Remote work is possible after 3–6 months, with travel to Ramsey County as needed.

Qualifications

• Bachelor's degree in social work, psychology, nursing, or related field required.
• Social work licensure required if degree is in social work.
• Experience with DD populations or willingness to complete Relias I/DD training; focus on adults with serious mental illness.

Responsibilities

• Provide client-centered case management for CADI/BI and DD waiver services; typical caseloads of 35–43 clients.
• Advocate for Fraser clients and connect to community resources; maintain accurate case notes and documentation.
