Fraser is seeking Case Managers to work with individuals in CADI/BI and DD waiver programs. You’ll advocate for clients, connect them to community services, and manage detailed documentation.

The role emphasizes compassion for people with developmental disabilities or related conditions and a strong focus on inclusion and culturally responsive practice. You will handle caseloads of approximately 35–43 clients, with 2–3 meetings per year per client, travel to meet clients 2–3 times weekly, and

Qualifications

• Bachelor's Degree in social work, special education, psychology, nursing, human services or other fields related to the education or treatment of people with developmental disabilities or related conditions.
• If degree is in social work, social work licensure is required.
• One year of experience with DD populations or Relias I/DD course (paid)
• Experience or interest in working with adults with serious and persistent mental illness
• Commitment to diversity, multiculturalism, inclusion, and culturally responsive practice
• Valid Minnesota Driver’s License with acceptable record
• Ability to pass DHS background study

Responsabilités

• CADI/BI WAIVER Case Managers provide client-centered case management to individuals with CADI/BI waivers; typical caseloads around 35 clients with 2–3 meetings per year.
• DD WAIVER & NON-WAIVER Case Managers provide client-centered case management to individuals with DD services; typical caseload around 43 clients with 2–3 meetings per year.
• Advocate for Fraser clients and connect them to community resources and services.
• Well organized with documentation and case noting skills.
