Job summary

Fraser is seeking Case Managers to support individuals on CADI/BI and DD waivers in Minnesota. You will coordinate care, advocate for clients, and help them access community resources, with strong documentation and client-success focus.

Caseloads average about 35 CADI/BI and 43 DD/non-waiver clients; meetings 2–3 times per year. Initial on-site work with fully remote option after 3–6 months; travel to meet clients 2–3 times weekly.

Qualifications

• Bachelor’s degree in Social Work or related field; licensure if Social Work degree.
• One year of experience in education or treatment of persons with developmental disabilities or related conditions.
• Completion of at least one course focused on developmental disabilities; DHS background check.
• Valid Minnesota driver’s license with acceptable record.

Responsibilities

• Provide care coordination and monitor health and safety to meet client goals.
• Intervene to resolve service gaps and advocate for resources.
• Maintain consistent client advocacy and connections to community resources.
