Fraser is seeking dedicated Case Managers to support individuals on CADI/BI & DD waivers. You will advocate for clients and link them to community resources, ensuring compassionate, client-centered service.

The ideal candidate handles about 35–43 clients, meets 2–3 times yearly, and brings commitment to diversity, inclusion, and accurate documentation. Remote work may follow onboarding, supporting Ramsey County clients from Minnesota.

Formación

• Bachelor's degree in social work, special education, psychology, nursing, human services or related field.
• If the degree is in social work, licensure is required.
• At least one year experience with DD populations or willingness to take Relias I/DD course (paid by Fraser).
• Experience or strong interest in working with adults with serious mental illness.
• Commitment to promoting diversity, multiculturalism, and inclusion with culturally responsive practice.
• Valid Minnesota driver's license with acceptable record.
• Ability to pass DHS background study.
• Excellent documenting and case noting skills.

Responsabilidades

• Provide client-centered case management services to individuals receiving CADI/BI waiver services and DD services.
• Advocate for Fraser clients and connect them to community resources and services.
• Manage caseloads; typically 35–43 clients, meeting 2–3 times per year.
• Maintain accurate case notes and documentation.
• Collaborate with multidisciplinary teams to coordinate services for clients.
