Fraser is seeking Case Managers to work with individuals on CADI/BI and DD waivers in Minnesota. You will advocate for clients, connect them to community resources, and document care meticulously.

Ideal candidates are organized, compassionate, and experienced with developmental or intellectually challenged individuals. Caseloads vary by program (CADI/BI about 35; DD about 43) with 2–3 client meetings per year.

Formación

• Bachelor's Degree in social work, special education, psychology, nursing, human services or other fields related to the education or treatment of people with developmental disabilities or related conditions.
• If degree is in social work, social work licensure is required.
• One year of experience with DD populations or Relias I/DD course (paid by Fraser).
• Experience or interest in working with adults with serious and persistent mental illness.
• Commitment to diversity, multiculturalism, and inclusion with culturally responsive practice.

Responsabilidades

• CADI/BI WAIVER Case Managers provide client-centered case management to individuals receiving CADI/BI waiver services, usually with about 35 clients on caseloads and 2–3 meetings per year.
• DD WAIVER & NON-WAIVER Case Managers provide client-centered case management to individuals receiving DD services, typically about 43 clients on caseloads with meetings 2–3 times per year; mix of children and adults.
