Job summary

Fraser seeks dedicated Case Managers to support individuals on CADI/BI & DD waivers. You will advocate for clients and connect them with resources in the community, while documenting outcomes and coordinating care. The role requires compassion, organization, and a focus on culturally responsive practice.

Experience with developmental or intellectually challenging populations is preferred. CADI/BI & DD caseloads, travel to client sites, and a Minnesota residence are expected.

Qualifications

• Bachelor's Degree in related field and licensure as required for social work.
• Experience or strong interest in working with adults with mental illness.
• Ability to pass DHS background study and hold a Minnesota driver's license.

Responsibilities

• Provide client-centered case management for CADI/BI and DD waiver services.
• Advocate for Fraser clients and connect them to community resources.
• Maintain clear case notes and documentation; manage caseloads of ~35–43 clients.
• Meet clients 2–3 times per year; travel to meet with clients as needed.
