Fraser is seeking CASE MANAGERS to support individuals on the CADI/BI and DD waivers in Minnesota. Your role includes care coordination, health and safety monitoring, and advocating for clients to access needed services.

Caseloads typically include CADI/BI and DD waiver recipients, with regular client meetings and active problem-solving to bridge service gaps. Applicants should have a relevant bachelor’s degree, one year of related experience, and be able to pass a DHS background check.

Formación

• Bachelor’s degree in Social Work, Special Education, Psychology, Nursing, Human Services, or related field; Social Work licensure if degree is in Social Work.
• One year of experience in education or treatment of persons with developmental disabilities (QDDP qualified).
• Course focused on developmental disabilities; Fraser provides this course free of charge.
• Minnesota Driver’s License with acceptable record.
• Ability to pass a DHS background study.
• Commitment to diversity, multiculturalism, and inclusion, culturally responsive practice.
