Job summary

Fraser is seeking Case Managers to support individuals on the CADI/BI and DD waivers, advocating for clients and helping them access community resources. The role emphasizes compassion, strong organization, and thorough documentation to support client success.

Ideal candidates have a bachelor’s degree in a related field (or Social Work with licensure) and experience with developmental disabilities. Work includes client visits and case notes, with a focus on inclusion and culturally responsive

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, or related field.
• If degree is in Social Work, Social Work licensure is required.
• One year of experience in the education or treatment of persons with developmental disabilities or related conditions (QDDP qualified).
• Completion of at least one course focused on developmental disabilities (Fraser can provide this course free of charge).
• Valid Minnesota Driver's License with acceptable record.
• Ability to pass a DHS background study.
• Commitment to promoting diversity, multiculturalism, and inclusion with a focus on culturally responsive practice, internal self-awareness, and reflection.

Responsibilities

• Providing care coordination, monitoring client health and safety needs, and ensuring services effectively meet client goals.
• Intervening as needed to support clients and resolve service gaps.
• Serving as a consistent advocate and connection to resources.
